Submit your chapter application through our secure portal. You will need at least 3 founding members, a proposed meeting schedule, and a brief charter statement.

- Minimum 3 founding members with active IFO4 membership
- At least 1 founding member must hold an active IFO4 certification
- Proposed meeting schedule (minimum quarterly)
- Brief charter statement outlining chapter goals and focus areas
- Commitment to IFO4 Code of Ethics and Chapter Governance Policy
- Designated Chapter Lead and Vice Lead
